E911 Disclaimer
Effective Date  7/1/2013

I, ___________________________________ (customer name) on this _______ (day) of ___________________ (month), __________ (year) hereby register the current location for my WiValley Digital Phone Service as:

Name: _____________________________________________________________

Address: ___________________________________________________________

City, State, Zip: ______________________________________________________

I understand that if I call 911 from the Digital Phones linked to my account, that the address I have registered in this document is where personnel will be directed and not necessarily the actual location of the phone at the time of the call.  I acknowledge that WiValley is not liable for any issues with 911 personnel being routed to an incorrect location for any reason.

Customer acknowledges that Digital Phone systems have a limitation with power outages, broadband outages, equipment failure, and equipment that is moved.  While WiValley required backup UPS systems for all customers using their Digital Phone solutions, WiValley is not responsible for the actual implementation or use of such devices.

Any change in address location must be submitted by completing a new E911 Disclaimer form and electronically sending the form to voip@wivalley.net at least 10 days prior to change being active in system.   Customer will receive emailed confirmation of the change of address being active in the system. If customer does not receive confirmation receipt, please contact WiValley at (603) 546-7200 to confirm the change in location has been received.

By using WiValley’s 911 service, the customer agrees that WiValley, it’s contractors, executives, members, customers, agents, employees, carriers, 911 providers, and anyone else associated with WiValley is not held liable for emergency calls failing, even if it is determined that it is the fault of WiValley or its associates. Customer further agrees that they will notify their family members, customers, contractors, agents, employees, associates, shareholders, partners, and anyone who may use the WiValley 911 service of our limitations and make customers agree to not hold WiValley or customer liable.

It is my responsibility as the customer to correct any e911 address discrepancies between the town and state.

If you do not agree to the terms and conditions set forth in these documents, you must cancel service immediately. Your use of and payment for the service indicates your acceptance of these terms and conditions and any amendments that may be made from time to time which will be maintained online at www.wivalley.net
